Optimizing Sexy Contact Form

Hi,
I am using Sexy Contact Form Pro and JCH Optimize Pro. (sexycontactform_pack_1.0.6_2glux.com)
The whole site works with all optimization options enabled, except the Sexy Contact Form.
The sexy form displays with all the text fields and the buttons and ends emails OK, but all the prettiness is missing.
So I guess the problem is JavaScript related.
Therefore, in JCH Optimize I have excluded the following .js file from the optimizing process:

sexylib.js
sexylib-ui.js
sexycontactform_unpacked.js
sexycontactform.js
sexy-mousewheel.js
jquery.iframe-transport.js
jquery.fileupload.js
jquery.fileupload-validate.js
jquery.fileupload-process.js
jquery.fileupload-angular.js
and
mootools-core.js,mootools-more.js,rokbox.js,

Still the form won't display correctly. What else should I do to get Sexy Contact Form working with site optimization?
